I permit my child to take part in the field trip described above. I have listed any physical or medical problems which might interfere with my child's participation in this activity. The undersigned, parent, or legal guardian of the Student identified above hereby authorizes and gives permission for the Student to attend the Field Trip identified above. The undersigned understands that the Student’s participation in this Field Trip is voluntary. The undersigned hereby releases and holds harmless the Second Baptist Church, organizer of the Field trip, and respective volunteers, representatives, successors and assigns, from any and all liability, cost, claim, damage, and expense, including reasonable attorney’s fees, whether foreseen or unforeseen, including any injury and cost of medical or dental treatment. Dear Parent/Guardian:

nGem (Next Generation Empowerment Ministry), will be going on a field trip to Fleischmann Planetarium and Rancho San Rafael Park. As a part of our re-launch, BLAST OFF, this field trip will provide your child with the opportunity to experience the wonder of the galaxies that were created by God and how they relate to us.

Your child should be dropped of at Second Baptist Church at 10:00 a.m. and picked up at 3:30 p.m.

We will provide Jimmy John’s sandwiches, chips and drinks for lunch.

If your child has any special physical or medical problems (e.g. allergies), please list below.
